Crisis Intervention, Respite & Supported Living
The decline in Respite Care in Worcestershire has been apparent to the clients, their families and sometimes to the residential homes that they live in for quite some time.
Whoever or whatever we are, we all need a break from our normal routines to ‘recharge’ our batteries and to take on new challenges and try new experiences, developing new ideas. For people with learning disabilities and difficulties in Worcestershire and the surrounding areas the homes providing such respite care have clearly declined over the last few years.
Rely Care Agency purchased ‘Constable House’ successfully in 2010 and since then has opened its doors to many who are seeking either short or long term respite. All of the individuals who come for respite care at Constable House will have a person centred care plan that will clearly outline their support needs.
There are a wealth of activities on offer to everyone during their stay at Constable House – a Monday night disco, a trip away for a weekend, a visit to the local shops. There are also opportunities for individuals to seek a work placement to help integrate them into society.
Rely Care has a supported living home that is ideal as it helps people to live as independently as possible. For many people who want to live independently, getting the right care and support is just as important as finding the right home. We have been supporting people for years, so we know what quality support looks like.
We can provide personal support to help you with things like getting dressed. We can also help you around the home, supporting with day to day chores and cooking a meal. We will also be able to offer support so people are able to go out in the community or to do a hobby that you particularly enjoy.
Support is available from a couple of hours a week to 24 hours a day, seven days a week.
Specialist Care
Rely Care Agency can provide experienced and specifically trained staff to help you maintain your quality of life while remaining in your own home.
We can provide specialist care services for individuals with learning disabilities/difficulties and complex needs as well as individuals living with Dementia or Alzheimer’s and support for a loved one needing assistance with day to day living.
A person centred care plan will be compiled by a member of Rely Care’s Management along with the individual requiring support and any members of family that wish to be involved, we feel that it is important to talk to as many members of each individuals family as we can when compiling a care plan so we can make sure that it is person centred. We will also seek advice and input as appropriate from other healthcare professionals.
Each care plan is tailored to the specific needs of each individual and will be updated on an ongoing basis.
Dementia Care
Living with dementia can be a bewildering experience, requiring significant lifestyle changes for the whole family. At Rely Care Agency our specialist dementia care services can provide the essential support you need during this time. Our Liaison Consultant has been trained at a Dementia Specialist 10 day training course run by the Department of Dementia Studies at Worcester University and our Managing Director has been trained at a 5 day training course. Care & Support
Rely Care Agency can provide experienced and specifically trained staff to help you maintain your quality of life while remaining in your own home. We create a specialised dementia care plan with you. This might include:
- Reminding or helping you to take your medicines to manage your symptoms
- Support with bathing and/or dressing to keep you looking your best
- Help to establish a regular exercise routine to improve mood and mobility and to strengthen muscles
- Making sure you have plenty of fluids and a balanced diet with enough fresh fruit and vegetables and fibre to keep you healthy and avoid constipation
- Assisting with routine household chores and shopping so your home life is as you would like it to be
- Companionship, or helping you to be involved in social activities.
Companionship Care
Our Companionship Care provides a wide range of comprehensive services which are delivered flexibly to meet the changing day-to-day needs that you or your loved ones may have.
Companionship Services include:
- Aid with morning wake up, bathing and dressing
- Assistance with clothing selection
- Assistance with walking
- Monitoring diet and eating
- Checking food expiry dates
- Making and serving meals
- Shopping
- Laundry and household chores
- Medication reminders
- Accessing the community
